Job Overview

An exciting opportunity has arisen for a GP to join the new Community Complex Conditions Service Multi-disciplinary team in Betsi Cadwaladr University Health Board (BCUHB). We are looking for someone who enjoys working as part of a skilled multidisciplinary team, supporting and collaborating. The team includes a Consultant Therapist, Clinical Psychology, Assistant Psychologists, Advanced Clinical Practitioners, Physiotherapy, Occupational Therapy, Pharmacy and support staff. The service supports people presenting with Long Covid, other post viral and fatiguing conditions, persistent physical symptoms, breathing pattern disorder and those who present frequently to services. The new Community Complex Conditions Service provides a real opportunity for the GP to get involved in the development of this service and shape the way we work with vulnerable groups who might otherwise fall through the gaps in services. We are an award winning, friendly and supportive team; who are research active, with a passion for co-production with our partners with Lived Experience.

We are currently looking for a GP to work with us. We are a pan North Wales service, and this role will be undertaken at 4 sessions per week on top of their current GP role, but within their current working practices.

Main duties of the job

The post holder will be required to undertake clinical work within the BCUHB CCC Service, working directly and indirectly with patients, families in outpatient clinics and in their own homes when necessary and staff members. As a key member of the Multi-Disciplinary team, the GP will provide clinical support to the MDT in the triage, assessment and holistic care planning of patients experiencing Post-Covid Syndrome including: Long Covid, Chronic Fatigue Syndrome/ME, Persistent Physical Symptoms, Breathing Pattern Disorder and those who frequently attend services. They will contribute to service evaluation and planning. In addition, they will provide support for Primary Care colleagues, through education and clinical leadership; as well participate in the ongoing service design and evaluation process.

Due to the evolving nature of the conditions covered by the service, regular ongoing review of the literature will be an integral part of the role, in order to inform the advice that is given to patients. These patient groups are often well informed and the successful applicant will need to maintain credibility with patients by being up to date with the latest research.

Education And Qualifications

Essential criteria

  • Fully qualified GP with GMC registration
  • Evidence of annual appraisal and revalidation (when appropriate)
  • General practice (Vocational Training Scheme) trained
  • On/eligible to be on a medical performers list
  • Enhanced CRB check
  • UK driving licence
  • Current CPR certificate
  • UK work permit (if required)

Desirable criteria

  • Evidence of continued professional development
  • Other Post Graduate Diplomas

Experience

Essential criteria

  • Successfully Completed General Practice Specialist Training
  • Appropriate range of previous experience in hospital posts
  • Clinical and Information Governance
  • Self audit and reflection
  • Organised and efficient in record keeping and completion of paperwork
  • Time management – being able to prioritise work and work under pressure
  • Computer literacy
  • Multi-morbid - Long Term Condition Management

Desirable criteria

  • Experience of working a variety of clinical software systems
